It included the following information: The person in charge is Nguyen Thi Thanh Tuyen - Head of the Department of Quality Management and Social Work (QLCL&CTXH); address 626, 30/4 Street, Ward 3, Tay Ninh. Quoting unit: Sau Na Furniture Store - TTNT; address: Khoi Trung, Cau Khoi, Duong Minh Chau.
In the list of goods, it is written: Medical cabinets, quantity 13, total price 45,000,000 VND; aluminum doors, quantity 5, total price 27,500,000 VND. Total price 72,500,000 VND. In addition, in this sales quotation, it is also clearly stated the payment method by bank transfer, bank name, account number, account holder, and advance payment of 36,250,000 VND.
At the end of the quotation document, there is a stamp with the full name and signature of the Director of Tay Ninh General Hospital, Phan Thanh Tam, and a red seal similar to the seal of Tay Ninh General Hospital.
After reviewing the document, Doctor Phan Thanh Tam said that the full name and signature were indeed his, the seal was no different from the seal of Tay Ninh General Hospital; the address was also that of the unit, but the person named Nguyen Thi Thanh Tuyen was not an officer or employee of the hospital.
Dr. Tam checked the hospital's Administration Department and confirmed that no one in the Administration Department had stamped the hospital's seal, nor his name and signature on this document. "I don't understand why this document has the hospital's seal, my name and signature," Dr. Phan Thanh Tam wondered.
The director of Tay Ninh General Hospital pointed out that there were many errors in the fake hospital document, such as: the hospital's official dispatch number was written as the official dispatch number of the Department of Health; the proposed content was not consistent, specifically, the fake document stated that the hospital's Quality Management and Social Work Department proposed to purchase items to serve office work, while this Department does not have the function of proposing the purchase of supplies.
The form of the fake document is also wrong, because no quotation document provides information about the account number for the buyer to transfer money. This account number is not of the hospital but of an individual, not an officer or employee of Tay Ninh General Hospital.
Doctor CKII Phan Thanh Tam affirmed that Tay Ninh General Hospital did not issue this document. He expressed his wish that through the mass media, the situation would be clearly announced, so that people would be aware, be vigilant, and avoid being taken advantage of by bad guys to defraud the hospital of goods and goods, in order to appropriate property.
The hospital leader affirmed: "If the hospital buys or sells equipment, there will be a contract and payment will be made with the hospital's finance and accounting department. The purchase and sale contract will be paid according to the hospital's account, not according to an individual's account."

However, in reality, in Khoi Trung Hamlet, there is no furniture store with the name as stated in the quotation, but only Thang - Sau Na Furniture Store, specializing in wholesale and retail of household plastic products such as cabinets, tables, chairs, mattresses, and sofas.
Ms. Dang Thi Ngoc Yen, owner of Thang - Sau Na furniture store, said that previously, an acquaintance working at the Provincial General Hospital introduced a woman who needed to buy some aluminum and glass cabinets. After that, the woman who introduced herself as Tuyen called the store to ask to buy some goods. The two parties agreed on the prices of the items to be bought and sold. This woman asked for Ms. Yen's account number and promised to transfer a deposit of 50% of the total value of the goods.
However, after waiting for a long time, the woman named Tuyen did not transfer the deposit and come to buy the goods. After that, this person called Ms. Yen to ask her to find and buy several hundred iron beds and mattresses. Realizing that this woman had a problem, Ms. Yen called her acquaintance to ask carefully about her relationship with the woman named Tuyen.
Ms. Yen's acquaintance said that she had never met the woman named Tuyen before, and had only communicated with her over the phone. Suspecting that this was a scam, Ms. Yen blocked her phone number, Zalo, and messages. Since then, the two parties have not contacted or bought or sold goods to each other.
This incident shows that there are many issues that need to be clarified by the authorities: Who is the woman who calls herself Nguyen Thi Thanh Tuyen - Head of the Department of Quality Management and Social Work? Why does this person have a price quote with the full name and signature of Doctor Phan Thanh Tam and a red stamp similar to the stamp of the Provincial General Hospital? Has this woman committed any fraud or appropriation of anyone's property?
