NDO - Doctors at the Central Dermatology Hospital have just treated a patient who initially had lesions that appeared as red, circular patches, but self-treated at home by applying unknown topical medications, and medications purchased online (no label, unknown ingredients), leading to widespread lesions all over the body.

A 17-year-old male patient from Quang Ninh was admitted to the hospital because of red, itchy lesions all over his body. The disease had been going on for 2 years. Initially, the lesions appeared as circular, itchy red patches on both arms. The patient went to the district hospital many times and was treated with topical medications. The lesions improved but then recurred intermittently. For a year now, the patient has been self-treating with unknown topical medications and medications purchased online (no label, unknown ingredients), and the lesions have spread throughout his body.

Upon admission to the hospital, through clinical examination, doctors found that the patient had red, circular, polygonal patches on the trunk, arms, and legs, with scaly skin, tending to heal in the middle and spread around; there were red papules and pustules on the chest and back; the patient was very itchy in the affected area. The results of some tests showed that the fungal filaments were segmented on the keratinocytes.

The patient was diagnosed with systemic fungal infection and prescribed treatment with Itraconazole 200mg/day, topical antifungal medication. After 5 days of treatment, the skin lesions improved, the patient was discharged from the hospital and continued treatment at home according to the prescription and was instructed on appropriate living and hygiene regimen to limit recurrence.

Dermatophytes are superficial fungal infections of the skin, including tinea corporis, tinea face, tinea cruris, tinea manuum, and tinea pedis. The main risk factors are related to raising or contact with animals; obesity and excessive sweating; use of alkaline soaps; frequent wearing of shoes, use of public bathtubs or swimming pools. The basic lesions are red, circular or polycyclic patches with scales, tending to heal in the middle, spreading around, and being very itchy. The disease responds well to treatment, but often recurs and may require long-term preventive treatment.

Dermatophytosis is a fungal skin infection caused by superficial fungi, commonly found in tropical countries with hot and humid climates. Dermatophytosis is a common fungal skin infection worldwide and can affect all ages. Diagnosis is mainly based on clinical symptoms and wet mount testing for fungi. Depending on the severity of the disease, doctors can use topical or oral medications alone or in combination. Response to treatment is quite good within 1-2 weeks of treatment.

Improper use of medication can make the condition worse and require longer treatment. Therefore, when patients show signs of illness, they should go to specialized medical facilities for timely examination and treatment.
