Changes in family health insurance premiums from July 1

According to current regulations, when participating in health insurance for a family group, the first person pays 4.5% of the basic salary. From July 1, the basis for paying health insurance will change.

The revised Law on Health Insurance 2024 will officially take effect from July 1, 2025, including changes in regulations on family health insurance premiums.

Specifically, Clause 11, Article 1 of the 2024 amended Law on Health Insurance stipulates that members participating in family health insurance in the fiscal year will have their premiums reduced as follows:

- The first person pays a maximum of 6% of the reference level;

- The second, third and fourth persons pay 70%, 60% and 50% of the first person's contribution respectively;

- From the fifth person onwards, the contribution is 40% of the first person's contribution.

The law explains that the reference level is the amount of money decided by the Government to calculate the contribution and benefit levels of a number of cases participating in health insurance as prescribed in this law.

Thus, the most obvious change in the basis for paying health insurance in this law compared to current legal documents on health insurance is that instead of using "basic salary", "reference level" is used.

The reference level stipulated in this law (which will take effect from July 1) is applied according to the basic salary level. In case of any changes in salary policy, the Government will decide on the specific reference level.

Regarding the basic salary in 2025, the National Assembly has agreed not to increase public sector salaries, pensions, social insurance benefits, monthly allowances, and preferential allowances for people with meritorious services. Therefore, the basic salary in 2025 will still be applied at the level of 2.34 million VND/month.

The regulation on the health insurance contribution level for household members of "maximum 6%" of the basic salary is not new (it has been stipulated in Article 1, Clause 7 of the Law on Health Insurance 2014 amending Article 13 of the Law on Health Insurance 2008).

According to current regulations in Decree 146/2018, the monthly family health insurance premium will be based on the basic salary, specifically:

- The first person pays 4.5% of the basic salary.

- The second, third and fourth person pay 70%, 60% and 50% of the first person's contribution respectively.

- From the fifth person onwards, the contribution is 40% of the first person's contribution.

There is currently no guidance on changes in regulations on health insurance contribution rates (according to the 2024 Law on Health Insurance), therefore, the first person in the household still pays health insurance at 4.5% of the basic salary (VND 1,263,600/year, equivalent to more than VND 105,000/month).

By the end of 2024, Vietnam will have 95.52 million people participating in health insurance, reaching a coverage rate of 94.2% of the population. Last year, the country had 186.2 million people receiving inpatient and outpatient health insurance treatment, with the proposed payment amount of nearly VND 143,000 billion (an increase of VND 18,685 billion compared to the same period in 2023). By the end of 2024, the Health Insurance Fund will have a surplus of VND 47,600 billion.

By the end of June 2024, the number of people participating in household health insurance was more than 23.48 million; the second largest among the 5 groups of people participating in health insurance (after the group paid by the state budget).
