Stomach pain for many days, went to the doctor and discovered the 'culprit' in the gallbladder

Báo Thanh niênBáo Thanh niên19/12/2024

For many days, Ms. LTBT (32 years old, living in Long An ) had pain in the epigastric region, pain in the right hypochondrium, and bloating of unknown cause.

The increasing pain made Ms. T. feel worried so she went to Xuyen A Long An General Hospital for a check-up.

At the General Surgery Department, after examination, doctors determined that the problem was a disease of the liver, gallbladder, and pancreas. Therefore, the patient was ordered to perform the necessary tests along with an abdominal MSCT (multi-slice computed tomography).

On December 19, specialist doctor Truong Minh Hieu, Head of Endoscopy Department, Xuyen A Long An General Hospital, said that on the MSCT image, there were signs of mild dilation of the left hepatic duct and common bile duct, the base of the gallbladder had many small stones and sludge accumulated in clumps measuring 30mm, the gallbladder duct had many small stones measuring 9mm, the gallbladder lumen had a few air bubbles with mild perfusion disorders of the liver parenchyma in the gallbladder bed, and fluid accumulation around the neck of the gallbladder.

The patient was diagnosed with gallstones, acute cholecystitis, and common bile duct stones. After consulting with related specialists, the doctors decided to perform two surgeries in the same operation. First, the team performed endoscopic retrograde cholangiopancreatography to remove the stones, followed by laparoscopic cholecystectomy.

After surgery, the patient was alert, had no abdominal pain, and all signs were normal. He was transferred to the General Surgery Department for further monitoring and is expected to be discharged in the next few days.

The patient underwent endoscopic retrograde cholangiopancreatography to remove stones, followed by laparoscopic cholecystectomy.

A dangerous complication of common bile duct stones is septic shock.

Doctor Hieu said that acute cholecystitis due to gallstones often occurs when stones get stuck in the neck of the gallbladder duct or large stones block the flow of bile, leading to infection. A dangerous complication of common bile duct stones is septic shock. If not treated promptly, it can lead to multiple organ failure.

Previously, patients with gallstones often had to undergo two surgeries. The first time, the doctor performed endoscopic retrograde cholangiopancreatography to remove stones from the common bile duct. After 3-5 days, the doctor performed the next surgery by endoscopic cholecystectomy. Currently, with the support of modern medical equipment and a team of highly skilled doctors, the above two surgeries can be performed in the same surgery. This not only helps patients shorten recovery time, but also minimizes risks from multiple surgeries and significantly saves treatment costs.

Through the above case, Dr. Hieu recommends that everyone should be alert to signs of colic pain in the upper abdomen or right hypochondrium, jaundice, diarrhea, high fever... When seeing these symptoms, you should go to a medical facility for examination to be diagnosed and have timely treatment.

Measures to prevent gallstones

Specialist Doctor 2 Nguyen Vu An, Head of General Surgery Department, Xuyen A Long An General Hospital, recommends some measures to help prevent gallstones such as:

- Add more fiber such as green vegetables, fruits and whole grains...

- Limit fat: Because cholesterol is a component of gallstones, you should limit fried foods or fast foods. In addition, you can use butter, vegetable oils and fish oils.

- Maintain a healthy weight : Obesity is a factor that increases the risk of gallstones. Therefore, it is necessary to combine a healthy diet and daily exercise to maintain a suitable body shape and weight.

- Avoid excessive dieting: Skipping meals regularly to lose weight is a wrong method, affecting bile secretion. This causes bile to not be excreted but to stagnate, leading to the formation of gallstones. Deworm regularly.

"In addition, because gallstones form silently and without symptoms, for prevention and early detection, people should have regular health check-ups to screen for diseases and detect gallstones," Dr. An recommended.
