On June 27, information from Ho Chi Minh City Eye Hospital, doctors from this unit and the City Ear, Nose and Throat Hospital have just coordinated to perform emergency surgery for a particularly dangerous injury case caused by a foreign object penetrating the eye socket to the left nasal cavity, with the risk of perforating the eyeball.

On June 26, Mr. TTĐ (42 years old, living in Dong Nai province) went to Ho Chi Minh City Eye Hospital for emergency examination and was hospitalized because his right eye was swollen and slightly bulging.

Through examination, the doctor discovered that the victim's right eye was inflamed and had a fractured orbital floor and ordered a CT scan for further diagnosis.

CT-Scan results detected a foreign object in the right eye socket penetrating the maxillary sinus through the septum to the left nasal cavity, the right eyeball was severely deformed due to compression of the foreign object, suspected of perforation of the eyeball.

Through medical history, Mr. TT D said that at around 3:00 p.m. on June 22, he was sitting at a friend's house in Bien Hoa, Dong Nai when a neighbor used plastic chopsticks to injure him in the right eye.

After that, Mr. D felt his eyes were blurred and swollen, and was given first aid and initial treatment at Thong Nhat Hospital (Dong Nai) by his family. However, his eyes became more and more swollen, so he went to Ho Chi Minh City Eye Hospital for examination.

Assessing the victim's serious injury, the doctors at the Eye Hospital consulted with the doctors at the ENT Hospital and agreed on a combined surgical plan, including exploring the eyeball, removing foreign objects from the eye socket through the nasal cavity, and reconstructing the floor and inner wall of the eye socket with Titanium mesh.

During the surgery, the ophthalmology team was extremely careful when removing the foreign object to avoid causing further damage to the eyeball. The foreign object was slowly pushed down into the maxillary sinus, passing through the floor and wall of the broken eye socket.

Immediately afterwards, the ENT team opened the nasal septum about 5mm, pushed the foreign object from the left nostril into the right nostril, then slowly pulled the foreign object out from the right nostril.

With smooth coordination, the doctors removed the foreign object, a piece of plastic chopstick, measuring 6 x 0.5cm.

After removing the foreign object, the doctors pushed all the herniated tissue in the maxillary sinus back into the eye socket and sutured the damaged structures, ensuring that the eye socket structure could recover its proper function.

The surgery lasted 3 hours and the victim's condition is now stable.

Doctor Bien Thi Cam Van, Deputy Head of the Department of Plastic Surgery - Neuro-Ophthalmology, Ho Chi Minh City Eye Hospital, said that it is important to determine the extent of damage to the eyeball to minimize further damage during surgery.

In addition to protecting the eyeball, during surgery, doctors must also remove foreign objects carefully, gently and meticulously to avoid any risk of further damage to the eye and maxillary sinus structure.

The process of patching the floor, the inner wall of the eye socket, and the maxillary sinus was also meticulously performed to ensure optimal recovery for the victim./.
