To change voluntary social insurance participation information directly, you go to a service organization or social insurance agency in the area, fill out a participation declaration, adjust social insurance and health insurance information (form TK1-TS issued with the procedure for collecting social insurance, health insurance, unemployment insurance, occupational accident and disease insurance; management of social insurance books and health insurance cards of Vietnam Social Security - Consolidated Document No. 2525/VBHN-BHXH dated August 15, 2023 of Vietnam Social Security), provide Citizen Identification information (changed) to adjust voluntary social insurance participation information.
