Successful surgery on a particularly dangerous eye injury

Doctors from the two departments of Ophthalmology and ENT at Ho Chi Minh City Eye Hospital coordinated surgery to remove a foreign object, a 6cm long plastic chopstick, that had penetrated from the right eye socket through the left nostril, saving the patient's life at the last minute.

On June 27, 2025, Ho Chi Minh City Eye Hospital successfully performed surgery on a particularly complicated and dangerous trauma case of "protruding right eye with foreign body in the eye socket penetrating the maxillary sinus through the septum to the left nasal cavity, with a risk of perforation of the eyeball due to the foreign body".

On June 26, 2025, Mr. TTĐ (42 years old, residing in Bien Hoa, Dong Nai ) came to the Ho Chi Minh City Eye Hospital for emergency examination and was hospitalized because his right eye was swollen and slightly bulging. The CT scan results showed that the patient had a foreign object in the right eye socket penetrating the maxillary sinus through the septum to the left nasal cavity, the right eyeball was severely deformed due to the foreign object pressing on it, suspected of perforation but vision was still quite high at 6/10.

Through medical history, Mr. TTĐ said: at around 3:00 p.m. on June 22, 2025, he was sitting at his friend's house in Bien Hoa, Dong Nai when a neighbor used plastic chopsticks to injure his right eye. Afterwards, Mr. D. felt his eyes were blurred and swollen. Therefore, Mr. D. received emergency care and initial treatment for his injuries at Thong Nhat Hospital (Dong Nai).

After an interdisciplinary consultation between the ENT and Ophthalmology departments and based on the CT scan results, the eyeball was severely deformed, with suspected serious damage caused by a foreign body. Accordingly, the interdisciplinary team of doctors agreed on a combined surgical plan, including exploring the eyeball, removing the foreign body from the eye socket through the nasal cavity, and reconstructing the floor and inner wall of the eye socket with a titanium mesh.

During the surgery, the ophthalmology team was extremely careful in performing the surgery after removing the foreign object to avoid any damage to the eyeball. The foreign object was slowly pushed down into the maxillary sinus, passing through the broken floor and wall of the eye socket. The ENT team opened the nasal septum about 5 mm, pushed the foreign object from the left nostril into the right nostril, then slowly pulled the foreign object out from the right nostril.

As a result, the foreign object removed was a piece of plastic chopsticks, measuring 6 x 0.5 cm. After removing the foreign object, the ophthalmology team found no damage to the eyeball. Accordingly, the team pushed all the herniated tissue in the maxillary sinus back into the eye socket and sutured the damaged structures, ensuring that the eye socket structure could recover its proper function. The surgery lasted 3 hours and the patient's condition is currently stable.

It is known that this is a very serious eye trauma case similar to the emergency surgery at the City Children's Hospital 3 years ago. This case is also the basis for endoscopic surgery of orbital cases according to the plan to develop new specialized techniques of the THTM-TKNK department such as endoscopic flooring, orbital decompression or endoscopic orbital tumors... at the Eye Hospital in the near future./.
