Successful treatment of patient with rare May Thurner syndrome

On July 2, Dr. Phan Huu Chinh - Director of Khanh Hoa General Hospital said that recently, with the professional support of Dr. Le Phi Long - Deputy Head of the Thoracic - Vascular Department of Ho Chi Minh City University of Medicine and Pharmacy Hospital, the Thoracic - Vascular Surgery Department of the Hospital for the first time deployed the technique of angiography and stent placement of the iliac vein, successfully treating a patient with May Thurner syndrome - a rare disease that causes narrowing and occlusion of the left iliac vein, leading to chronic deep vein thrombosis (blood clotting) of the left lower limb.

Patient NTT (female, 33 years old, Nha Trang ward) was admitted to the hospital with severe left leg pain, which seriously affected her daily activities and quality of life. Previously, the patient had been diagnosed and treated for left iliotibial vein thrombosis with anticoagulants and venous compression stockings, but the pain showed no signs of improvement.

After conducting a clinical examination combined with modern imaging diagnostic equipment (vascular doppler ultrasound and MSCT of the lower limbs with contrast), the doctors of the Department of Thoracic and Vascular Surgery determined that this was a rare case of May Thurner syndrome; performing vascular intervention for treatment is very complicated and difficult. However, with the support of leading doctors from the University of Medicine and Pharmacy Hospital, the team of doctors of the Department of Thoracic and Vascular Surgery decided to perform endovascular intervention, angioplasty combined with stent placement of the left iliac vein to restore blood flow and relieve compression for the patient. The intervention procedure took place in 1 hour, was successful and left no complications. The patient was discharged after 2 days of intervention, with much less pain and swelling in the leg, and was prescribed anticoagulants for the first 3 months to prevent venous restenosis due to thrombosis.

According to Dr. Le Viet Huan - Head of Thoracic and Vascular Surgery Department, Provincial General Hospital, May Thurner syndrome is one of the rare causes of deep vein thrombosis in the femoral iliac vein. The syndrome occurs when the right iliac artery crosses and compresses the left iliac vein, slowing the flow of blood from the legs back to the heart, thereby increasing the risk of deep vein thrombosis at the site of the compressed iliac vein.

The successful implementation of the technique of angiography and stenting of the iliac vein - a very difficult technique right at the Provincial General Hospital, shows that the skills of the hospital's medical staff are increasingly improved, a stepping stone towards becoming a hospital in the South Central Coast region.
