Testicular torsion but arriving at the hospital too late, one testicle became necrotic, the doctor had to remove it to avoid the risk of infection, systemic poisoning that could lead to death.

According to medical history, Mr. TNT (50 years old, in Dong Nai ) had severe pain in the right scrotum for more than 5 days and was taken to a local hospital. The doctor suspected that he had spermatic cord torsion or orchitis and prescribed surgery, but the patient did not agree.

After that, Mr. T. went to Tam Anh General Hospital in Ho Chi Minh City for a re-examination. At the hospital, the doctor performed an ultrasound and discovered that the patient had a twisted spermatic cord and that his right testicle was necrotic, so he was required to have immediate surgery.

Arriving late at the hospital, Mr. T's testicle was necrotic so the doctors had to remove it.

Surgery to remove one testicle

“If testicular necrosis lasts too long and is not treated urgently, there is a risk of infection, creating toxic substances that cause systemic poisoning, endangering life,” said Master - Specialist Doctor 2 Pham Thanh Truc, Department of Urology, Tam Anh General Hospital, Ho Chi Minh City.

The team of doctors performed surgery to remove one of the patient's testicles. The doctor then closed the patient's incision with absorbable sutures.

Doctor Truc said that testicular torsion is a phenomenon of twisting the blood vessels and vas deferens - important components that transport sperm, causing blockage of blood circulation, cutting off the blood supply to the testicles. This is an emergency condition, requiring quick surgery to prevent permanent damage to the testicles.

"Golden time" to save testicles

The first 6 hours after the onset of testicular torsion symptoms are the “golden time” to save the testicles and restore blood circulation. If the torsion is left for too long, it will cause a lack of oxygen and nutrients to the testicular tissue, leading to testicular damage and necrosis, affecting fertility and hormone production in men.

According to Dr. Truc, testicular torsion often occurs in men, affecting 1 in 4,000 boys under 25 years old, accounting for up to 25% of acute scrotal pathologies in pediatrics, according to the US National Institutes of Health . The cause may be due to a congenital cord with an abnormally long structure, not firmly attached to the testicle, due to genetic factors or due to strong physical activity causing impact, strong movement, collision or trauma to the groin area can also cause testicular torsion. "In Mr. T's case, due to strong physical activity, causing trauma to the groin area, the right testicle was twisted for 5 days, causing necrosis, so the only way left was to remove it," said Dr. Truc.

Dr. Truc also noted some symptoms of testicular torsion in men such as sudden severe pain in the scrotum, accompanied by testicular swelling, nausea and vomiting, abdominal pain, the twisted testicle tends to be higher than the other testicle, the scrotal skin changes color from red, purple, brown, black (due to anemia).

If testicular torsion is detected, men need to go to the hospital immediately for examination and timely treatment during the "golden time", avoiding the risk of serious damage to the testicles, affecting reproductive function in men.

In addition, men need to avoid strenuous activities or strong impacts in the groin area, and have regular health check-ups to promptly detect abnormalities in the body.
