Emergency treatment for man in coma due to electric shock while repairing water tank

On July 7, the Central Hospital for Tropical Diseases informed that the hospital's doctors had just received Mr. NPT (66 years old, in Hanoi ) in critical condition, in a deep coma, suspected to be due to electric shock.

The patient was admitted to the hospital in a deep coma, with no pulse or blood pressure, both pupils dilated to 5mm, and no light reflex. There were many burn marks on the patient's body on both knees, the back of the hands, around the navel, and the tip of the penis - typical signs of suspected electric shock.

According to family members, about 15 minutes before being discovered, the patient was repairing a water tank on the rooftop when he suddenly lost consciousness and was discovered by family members and taken to the hospital. Based on clinical signs, doctors diagnosed the patient with cardiac arrest due to electric shock.

Upon admission, the doctors immediately performed CPR: chest compressions, adrenaline injection, endotracheal intubation, and two electric shocks. After 30 minutes of intensive CPR, the patient regained his pulse. However, blood gas results showed that the patient was in a state of severe metabolic acidosis, high blood potassium, and had to maintain two vasopressors due to shock after CPR.

After the pupils showed signs of constriction, the doctors decided to perform advanced resuscitation with measures such as mechanical ventilation, continuous blood filtration, and placing the patient on a controlled hypothermia system to protect brain function. The patient is currently receiving intensive treatment in the Emergency Department.

Doctor Le Son Viet said this is a case with a risk of very severe neurological sequelae after cardiac arrest. However, the patient is currently making certain progress, the pupils have returned to normal and have reflexes, the vasopressor has been stopped and the dialysis has been stopped.

To prevent electric shock accidents in daily life, Dr. Viet reminded people to be especially careful when repairing electrical equipment or systems in the home. Before operating, the power source must be completely disconnected by turning off the circuit breaker or circuit breaker. Repairs should only be done with dry hands, standing on a dry surface and using specialized tools with insulation such as gloves, screwdrivers, and insulated pliers. People should absolutely not climb onto the roof, water tank or high places related to the electrical system when safety is not guaranteed. Old electrical equipment, showing signs of damage or leakage, should be checked and replaced promptly. In addition, regular inspection and maintenance of the electrical system is also an important solution to prevent electric shock accidents in the home.

Doctor Viet recommends that in case of electric shock, you need to stay calm to handle the situation properly. First of all, everyone must quickly turn off the power source or use an insulating object such as a wooden stick, broom handle... to separate the victim from the electric current, absolutely do not touch the person being shocked directly. Call 115 immediately and check the victim's reaction.

In case the victim stops breathing and has no pulse, chest compressions and artificial respiration should be performed, if the support person has been trained in the technique. For conscious victims, they should be kept still, kept warm and monitored continuously until medical staff arrives. Absolutely do not pour water on the victim and limit movement in case of suspected spinal injury./.
